Girls/Boyz First!! Mentor Agreement

As a Mentor in the Girls/Boyz First!! Program, I __________________

agree: (Name)

· To attend a training session or one on one orientation before beginning

· To be on time for all activities

· To notify mentor coordinator or mentee in a timely fashion if I am unable to attend a planned mentee meeting or mentor meeting or gathering

· To engage in the mentor relationship with an open mind

· To keep discussions about my or with my mentee confidential

· To ask the mentor coordinator when I need assistance or clarity about something

· To notify the mentor coordinator of any change of address and telephone number

· To notify the mentor coordinator if I wish to terminate my activities (or make changes) with Girls/Boyz First!! Mentoring Program

· To adhere to the ethical and conduct guidelines when working with mentees

· To act and communicate professionally and in the best interests of our mentees

· Not to use alcohol, tobacco, or drugs in the presence of my mentee
